This agreement is made between Plumbers Elite, located at 74 Main Ocho Rios, in the parish of St. Ann (the "Service Provider"), and the Client identified in the "Client Name" and "Client Address" fields below (the "Client").
On the date specified in the "Agreement Date" field below, Plumbers Elite agrees to provide annual maintenance services for the Client under the following terms:
1. Commencement Date
This agreement shall commence on the date specified in the "Commencement Date" field below (hereafter referred to as the "Commencement Date") and expire on the date specified in the "End Date" field below.
2. Services Agreement Outlined
(A) One annual general servicing of all plumbing systems, as outlined in the selected package (refer to the service details below).(B) The Client is entitled to seven (7) auxiliary services annually, which include labor and parts, with parts covered up to a maximum value of $5,000 per service.(C) If materials required for a service exceed the $5,000 limit, the Client will be responsible for the additional costs. Should all the parts included in the package be used on one job, the Client will bear the cost of all subsequent parts for the remainder of the contract period. However, labor will remain as agreed.(D) The Client must sign off on each service completed. If unavailable, an approved representative must be appointed to sign on their behalf.(E) During servicing, the Client or an approved representative must be present to observe. If neither is available, the Client must submit written approval for the service to proceed in their absence.(F) Plumbers Elite shall not be held liable if a service cannot be performed due to the Client or their representative’s absence. In such cases, rescheduling will be done at the convenience of Plumbers Elite.(G) The selected package is limited to a specified annual value equivalent to the amount paid under this agreement.
3. Payment Arrangement and Service Details
The Client agrees to pay the annual fee specified in the "Fee Amount" field below. This fee represents a one-time annual payment for the services outlined.
4. Assurance Servicing Breakdown
(A) Beyond the annual general servicing, the Client is entitled to seven (7) response services per year for plumbing issues covered under the agreed package.(B) If services exceed this limit, the Client will bear all labor and parts costs.(C) Unused services will not carry forward to the following year. Packages are subject to amendments based on the Client’s needs.(D) The following basic repairs are included but limited to the specified parts below. Parts from previous contracts are non-transferable:
Three (3) angle valve repairs or replacements.
Replacement of up to four (4) flex hoses.
Two (2) galvanized lock-off valves.
Two (2) hose bib repairs or replacements.
Shower mixer washer, cartridge, or seat replacement (cartridges not included).
Two (2) inlet valve replacements.
Two (2) outlet valve replacements.
Two (2) flush handle replacements.
Two (2) flapper ball replacements.
5. Response Time for Single Service Requests
Non-emergency requests will be addressed within five (5) business days, subject to team availability.
Emergencies will be treated with urgency. Clients may send photos or videos of the issue to assist with assessment.
6. Termination of Contract
A one-month (30 working days) written notice is required from either party to terminate the contract.
Plumbers Elite reserves the right to decline service or terminate the contract with proper notice.
If the contract is terminated after four (4) months, the full amount is non-refundable. For termination within four months, 50% of the payment will be refunded within ten (10) working days.
7. Renewal of Contract
Contracts are renewed annually, and fees may be adjusted upon renewal.
Re-evaluation of the property may be required before renewal, particularly if significant changes to plumbing infrastructure have occurred.
